OIG form / page is not working

I am trying to apply for an OIG to receive a key but the page itself is not working, I am receiving CORS errors on submission.

Any other way to apply for OIG?

Hi @pepeday, sorry to hear you’ve been having an issue, have you tried accessing the form via a different browser? I’ve had a few people check the form and we can’t see a common issue, thanks
